Position Overview: We are seeking a dedicated Nurse Practitioner to join our care team in New York. You will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. Key Responsibilities:
In-Home Assessments (IHA): Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
Diagnosis and Treatment: Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
Chronic Disease Management: Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
Medication Management: Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.
Preventive Care: Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
Patient Education: Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
Primary Care Provider of Record: Serve as patients primary care provider.
Documentation: Maintain accurate and up-to-date electronic health records (EHR) to track patient progress, treatment plans, and interventions.
Bedside Manner: Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
Follow-Up: Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
Quality Assurance: Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
After-Hours On-Call: Participate in after-hours on-call and conduct longitudinal care and IHA visits as needed.
Travel: Drive to client and/or patient sites within a designated area.
Flexibility: Other duties as assigned based on business need.
Qualifications:
Active, unrestricted current licensure as a Nurse Practitioner in the state of New York.
Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
Obtain autonomous provider status within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
Active drivers license; licensed for a minimum of 3 years without restrictions; no DUI or other felony driving conviction in the past 7 years.
Current Basic Life Support Certification for health care providers.
Active DEAlicense.
Strong clinical assessment and diagnostic skills.
Excellent communication and patient education abilities.
Proficiency in electronic health record (EHR) systems.
Preferred Qualifications:
HEDIS/STAR measures experience
Homebased visit experience
HCC documentation experience
General Physical Requirements:
Ability to complete physical requirements of the job including navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment.

Schedule:
Working Hours: Our standard business hours are from Monday to Friday, 8am 5pm, with a one-hour lunch break.
Weekly Visits: Our minimum workload is 25 visits per week.
