*Conviva Senior Care Centers* is seeking a Board Certified/Board Eligible Advanced Practice Provider for a position in Corpus Christi, TX.

*A day in the life of a Conviva provider:*

* See 12-15 patients per day. Opportunity to spend more time with patients
* Thrive in a value-base care model and improve patient’s quality of life
* See a variety in your caseload, from preventative care to chronic conditions
* Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

*Your Accomplishments are Valued:*

* Active, unrestricted NP/PA license in the appropriate state
* Current certification as an NP or PA
* Current DEA license or ability to obtain a DEA license
* This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
* Must be passionate about contributing to an organization focused on continuously improving consumer experiences

*Preferred Qualifications:*

* *One year experience as an NP or PA*
* Familiar with Value Based Care model
* Outpatient Senior Care Experience
* Familiar with HEDIS measures

Pay: $110,000.00 - $130,000.00 per year
