Job Description

Department Overview : Commonpoint Resettlement & Integration Department consists of initial resettlement, social integration and employment services programs serving refugee and other humanitarian migrant populations. The R&I Department has office locations in midtown Manhattan and Elmhurst, Queens.

POSITION SUMMARY:

Commonpoint seeks a PC Case Manager, Gap Services to provide resettlement, integration and other social inclusion services to refugee and other humanitarian migrant clients through the Preferred Communities (PC) Refugee and Entrant Assistance (REA) and Matching Grant (MG) programs. The responsibilities of the Case Manager will include two main components: providing initial resettlement services to recently arrived Cuban/Haitian entrants through the PC Cuban/Haitian Initial Services (CHIS) program, as well as providing “gap services” to eligible clients of any nationality. This position will report to the Family Integration Program Manager. Fluency in Haitian Creole is required; fluency in French is preferred.

DUTIES AND RESPONSIBILITIES:

  • Receives comprehensive training on the Preferred Communities program (especially the CHIS and gap services components), Matching Grant (MG), and other programs for which clients are eligible.
  • Conducts outreach to identify eligible clients, especially Cuban/Haitian entrants, including through outreach to sponsors, and local community-based organizations.
  • Performs initial screening to identify needs, assess whether clients are in safe situations and prepare a family self-sufficiency plan.
  • Refers eligible individuals to internal and external resources, including employment services (MG or Refugee Social Services Program), intensive case management, mainstream public benefits, refugee health screenings, English as a New Language (ENL) providers and more.
  • Assists with applications for Social Security cards, Employment Authorization Documents, driver’s licenses, and school enrollment.
  • Conducts ongoing check-ins and follow-ups with CHIS-eligible individuals for up to 90 days after initial intake and assessment; conducts follow-ups with gap services clients as needed.
  • Provides case management for Cuban/Haitian entrants enrolled in MG for the duration of the 8-month program period.
  • Screens for direct client financial assistance eligibility in accordance with Commonpoint’s basic needs direct assistance rubric, facilitates disbursement, and conducts all necessary follow-up and associated financial reporting.
  • Conducts community outreach and coordination in order to connect newly arrived clients in the New York City area to resources or organizations that can support their basic needs as possible.
  • Collaborates with Commonpoint’s Volunteer Services team to support clients through in-kind donations or volunteer career mentorship, ENL tutoring, or interpretation.
  • Completes case notes and required assessments in online database (Apricot) in a timely and consistent manner.
  • Maintains comprehensive case files for each client enrolled in CHIS, MG and gap services, ensuring client confidentiality and protection of personally identifiable information.
  • Coordinates with other Commonpoint case managers providing gap services to ensure that all gap services documentation is accurately updated in Apricot on a monthly basis.
  • Oversees volunteer interns as needed.
  • Other duties as assigned by the supervisor.


QUALIFICATIONS & REQUIREMENTS:

  • Undergraduate degree required.
  • Fluency in Haitian Creole is required; fluency in French is strongly preferred.
  • 1-2 years of relevant experience.
  • Previous experience in refugee resettlement, social services and/or immigration law strongly preferred.
  • Experience with low-income, limited English, vulnerable immigrant populations preferred.
  • Knowledge of NYC social service organizations and resources for basic needs of low-income New Yorkers desired; familiarity with NYC Haitian grassroots organizations helpful.
  • Ability to supervise interns and volunteers as needed.
  • Strong oral and written communication skills.
  • Ability to prioritize and multi-task several urgent projects in a fast-paced environment.
  • Excellent computer skills, including experience with Salesforce, Microsoft Office and Google applications (Gmail, Calendar, Contacts, Docs, Sheets, etc.).
