DART ADVOCATE/PROGRAM COORDINATOR (Home Visiting Initiative)

Reports to: The Social Service Project Manager the Delta Assault Response Team (DART) Program for Issaquena, Leflore, Sharkey, Warren, and Yazoo Counties.

SUMMARY: The DART Advocate/Program Coordinator is responsible for assisting in the implementation and operations of services associated with Delta Health Alliance programs.

MIMIMUM QUALIFICATIONS:
  • This position requires a bachelor's Level Social Work degree with two (2) years of experience as a social worker in the Mississippi Delta.
  • Must have experience providing guidance and services to consumers and have familiarity with local health and social resources.
  • Three years or more residence in the county to be served and a license in social work is preferred.
  • Excellent verbal and written communication skills with computer literacy required.
  • Valid driver's license and automobile insurance
ESSENTIAL DUTIES
  • Assist DHA families in the program counties by providing advocacy services and support to survivors of relationship violence, stalking, and sexual assault or abuse and their dependents.
  • Establish additional partnerships in the specified counties to enhance the services provided by the program.
  • Collaborate with clinics, law enforcement, courts, hospitals and social resource coordinators and DHA staff and partners with primary responsibilities related to family dynamics.
  • Refer clients to other agencies and resources and advocate directly with other agencies as needed to help client access necessary services. Maintain service partnerships with referral agencies.
  • Assess immediate crisis needs of clients and address appropriately. Manage incoming calls and walk-ins and complete necessary documentation. Provide crisis intervention, emotional support, legal advocacy and court accompaniment for DART victims.
  • Provide information about rights and options, assistance with restraining order paperwork, shelter placement and resource navigation.
  • Educate clients about dynamics of domestic violence and sexual assault and options/resources available.
  • Complete and maintain client records that document services provided.
  • Continue to follow DART clients for a period of 2-6 months to ensure that they continue to have their needs addressed and maintain access to services needed.
  • Assist families with benefit enrollment while enrolling participants and families in ETO and maintaining ETO.
  • Maintain confidentiality and ensure compliance with HIPAA regulations.
  • Assist with the development of reports and other required documents as needed.
  • Coordinate the available resources to serve families, including follow-up and referral services.
  • Participate in staff meetings, case conferences and other meetings as needed.
  • Provide crisis line and office coverage. This position requires the advocate to be on-call on a rotating basis.
